Sharing medical data: only with your consent!

We will pass on the results of your test at Diagnostiek voor U to the healthcare provider who requested the test. This is usually your GP or obstetrician. In some cases, it may be necessary for other healthcare providers to access your data as well, such as if you unexpectedly have to go to another doctor or pharmacy. We may only share these data if you give us permission to do so.

Why is it important to allow us to share your data?

  • When all healthcare providers involved in your treatment have the same data:
    - you do not have to tell the same story over and over again.
    - you do not have to take the same tests time and time again.
  • If you are on holiday elsewhere in the country and forget your medication, a local pharmacy will easily be able to help out
  • In emergencies, the doctor on duty at the GP surgery or emergency room will be able to access your details so that they can treat you quickly, properly and safely.

What data do we share?

Our client records contain your personal data and the results of tests which you have had performed by us, including blood test results and ECG results. We share this data only if you have given us permission to do so and only if it is necessary for your treatment.

Who is allowed to access your data?

Only healthcare providers who are currently treating you may request your data, provided they need your data for your treatment. This includes GPs, pharmacies, medical specialists and their stand-ins. Health insurers, company doctors and employers are not allowed to access your data.

Privacy

Your privacy is very important to us. We only use secure technologies to exchange your data, possibly in combination with secure networks that can only be accessed by healthcare providers who meet strict security requirements.

Data exchange improves services

We work with a research agency to conduct client satisfaction surveys. These surveys are not anonymous and we need your consent to conduct them.

Consent

It is up to you to choose whether or not to consent to us sharing your data. You can also set a time limit for your consent. You may change your choice at any time.

You can notify us of your choice in three ways:

  • At the counter. Our staff member will then ask you for your ID and enter your choice into our client records.
  • Via MijnDiagnostiekvoorU, our client portal.
  • Via the 'Share data' form. You will find this form at the bottom of this page. You can download this form, complete it and hand it in at the front desk or give it to a member of our staff after a blood test. You can also send the form to: Diagnostiek voor U, Antwoordnummer 10060, 5600 VG Eindhoven. No postage is needed.

Giving Consent for Children or Legally Incapable People

If you have a minor child or are the official caregiver or guardian of a family member or acquaintance, you can give or withhold consent on their behalf.

You can update your choice quickly, easily and safely on MijnDiagnostiekvoorU. Our receptionist can also change your choice and update our client records right away.

No consent or no choice

If you do not consent to us sharing your data, withdraw your consent or do not wish to make a choice, keep in mind that other healthcare providers won’t be allowed to see your data.
