Giving consent for minor children or legally incapable people

Is your child under 16 years of age or if you are the official caregiver or guardian of a family member or acquaintance, you can indicate whether their data may be shared on their behalf.

Children

  • up to 12 years: only the parent/caregiver/guardian can give consent. Consent given this way automatically lapses when the child turns 12.
  • 12 to 16 years: both the parent/guardian and the child pass on their choice. Consent given this way automatically lapses when the child turns 16.
  • From 16 years: only the child gives consent.

Legally incapable person

  • If the client cannot be considered capable of looking after their own interests (legally incapable), their legal representative may give consent on their behalf.

Disclosing choice

You can notify us of their choice via MijnDiagnostiekvoorU, our client portal. For this, you need to log in with the DigiD of the person concerned. You can also complete the Share Data form and send it to us.
